Quad Mix Dosage: Test Dose, Side Effects and Safe Use

Quad mix dosage isn’t one-size-fits-all, and it’s never guessed at home. At Bliss MD, every new patient starts with the smallest dose during a supervised test dose in our office. From there, we titrate up slowly until we find what works.

Why There's No Standard Quad Mix Dosage

Men respond to injections very differently. Blood flow, nerve health, other medicines and the cause of ED all play a part. Two men can need very different amounts for the same result.

Your dose has two parts, and both are set for you:

  • Strength: how concentrated your mix is, chosen by your physician
  • Amount: how much you draw up for each injection, found during titration

UCSF Health notes that finding a dose that works usually takes some adjustment early on. The goal is the lowest dose that gives a firm erection that fades on its own.

Your Supervised In-Office Test Dose

Every new patient at Bliss MD gets a supervised test dose in our office, and it’s where your quad mix dosage begins. Here’s how it goes:

  1. Review. We go over your health history and medicines once more.
  2. Smallest dose first. We start with the lowest amount of your prescribed mix.
  3. Watch and wait. We stay with you and see how firm the erection gets and how long it lasts.
  4. Slow titration. If needed, we raise the dose in small steps, never in big jumps.
  5. Training. We teach you how to dose and what to avoid, step by step.

If an erection lasts too long during this phase, we treat it right in the office before you go home.

How Your Dose Is Adjusted Over Time

Your first dose is a starting point, not a final answer. As we learn how you respond at home, we fine-tune your quad mix dosage: the amount, the strength or both.

Let us know if:

  • The erection isn’t firm enough for sex
  • It lasts longer than you’d like
  • The injection causes an ache that bothers you
  • You start a new medicine

How Quad Mix Is Injected

You’ll learn hands-on in the office, using either a standard syringe or an auto-injector, whichever you prefer. We provide syringes, alcohol pads and a sharps container. This list is a reminder, not a substitute for training:

  • Wash your hands, then wipe the vial top and the skin with alcohol.
  • Draw up only your prescribed quad mix dosage.
  • Inject into the side of the shaft, not the top or underside, and avoid any vein you can see.
  • Switch sides each time to help prevent scar tissue.
  • Press on the spot for a few minutes to limit bruising, longer if you take a blood thinner.
  • Drop the needle in the sharps container.

Most men find it easier than they expected. UCSF Health notes that proper training lowers the risk of pain, bruising and scarring.

How Often You Can Use Quad Mix

At Bliss MD, the general guide is no more than 2–3 times a week, and your physician may set a lower number. These rules apply to everyone:

  • Never inject more than once in 24 hours.
  • Don’t re-inject if a dose doesn’t work. Wait at least 24 hours and call us.
  • Space your doses out across the week.

Memorial Sloan Kettering gives its own patients the same 24-hour and no-repeat rules. Spacing doses also helps protect the tissue over the long term.

What to Avoid With Quad Mix

Some combinations raise the risk of an erection that won’t go down. Don’t use quad mix at the same time as:

  • Sildenafil (Viagra)
  • Tadalafil (Cialis)
  • PT-141

Also tell us before you start if you take or have:

  • Blood thinners
  • Nitrates or other blood pressure medicines
  • Sickle cell disease, leukemia or multiple myeloma
  • A penile implant, Peyronie’s disease or scarring

Bring a full list of your medicines and supplements to your first visit.

Quad Mix Side Effects

Most side effects are mild, and a well-set quad mix dosage is the key to minimizing side effects.

More common:

  • A brief pinch from the needle
  • A dull ache afterward, often from the alprostadil
  • A small bruise at the injection site

Less common:

  • Light-headedness or flushing
  • An erection that lasts too long

With long-term use:

  • Firm areas, small lumps or scar tissue in the penis, which can lead to curvature

Tell us about any new lump, curve or pain at your next visit, or sooner.

An Erection Lasting More Than 4 Hours Is an Emergency

A prolonged erection, called priapism, is the most important risk of injection therapy. If an erection lasts more than 4 hours, go to the nearest emergency room. Don’t wait to see if it goes away.

Before that point, ice can help, because cold narrows the blood vessels, and sometimes an antihistamine. We go over both at training. Neither one replaces the emergency room.

Why the urgency? Trapped blood starves the tissue of oxygen, and the AUA priapism guideline calls for fast treatment. Careful titration makes priapism uncommon, but every man on injections should know this rule.

Storing and Traveling With Quad Mix

  • Know your source. Your mix is prepared by U.S.-based, FDA-inspected compounding pharmacies.
  • Follow your label. Quad mix is usually kept in the refrigerator, away from light.
  • Put it back promptly. These medicines slowly lose strength when they aren’t kept cold.
  • Check before each use. Skip it if it looks cloudy or has particles.
  • Travel smart. Keep it cool in a small insulated case, and pack your sharps safely.

When to Call Us

Call the clinic at (630) 289-0440 if:

  • Your quad mix dosage isn’t working, or works too well
  • You notice a new lump, curve or ongoing pain
  • Bruising is large or doesn’t fade
  • You start a new medicine, or your health changes

Quad Mix Dosage FAQs

There isn’t one. Your strength and amount are set for you. Every new patient starts with the smallest dose at a supervised test dose, and we titrate up slowly from there.

We give a small first dose in the office and watch how you respond, then raise it in small steps. If an erection lasts too long, we treat it before you leave.

Usually about 5 to 15 minutes. Arousal and your dose affect the timing.

With a well-set dose, often about 30 minutes to an hour. It should fade on its own.

Bliss MD generally recommends no more than 2–3 times a week, and never more than once in 24 hours. Your physician may set a lower limit.

Don’t inject again. Wait at least 24 hours, and call us so we can check your technique and adjust your dose.

Not at the same time. Combining quad mix with sildenafil, tadalafil or PT-141 raises the risk of a prolonged erection.
