Quad mix injections in Chicago give men another path when ED pills haven’t worked well enough, or aren’t a good fit. At Bliss MD, Dr. Anand Thakkar prescribes quad mix only after a full evaluation. Every new patient then starts with a supervised test dose in our office.

What Are Quad Mix Injections?

Quad mix is a compounded ED medicine given with a very fine needle into the side of the penis. Doctors call this intracavernosal injection therapy. The American Urological Association lists injection therapy among the established treatments for ED.

The mix is built on three well-studied medicines, with a fourth ingredient added. Each of the three core medicines works on a different part of an erection:

Papaverine Relaxes the smooth muscle of the erectile tissue
Phentolamine Helps keep the small arteries open
Alprostadil (prostaglandin E1) Widens the arteries so more blood flows in

Bimix uses the first two. Trimix uses all three. Quad mix uses all three plus a fourth ingredient, compounded together in one vial. Because the medicine works right in the penis, it doesn’t depend on the same pathway as ED pills. That’s why it can help some men whose pills fall short.

Is Quad Mix FDA-Approved?

No. Compounded quad mix is not an FDA-approved drug, and neither are compounded bimix and trimix. Each vial is prepared for one patient’s prescription instead of being mass-produced. The FDA’s compounding Q&A explains how compounded medicines differ from approved ones.

Who Quad Mix for ED Is For

Quad mix for ED is often considered when:

  • ED pills haven’t worked reliably, even at a full dose
  • Pills cause headaches, flushing or other effects you’d rather avoid
  • You take a medicine that shouldn’t be paired with ED pills
  • Trimix helped, but not quite enough
  • ED is tied to diabetes, blood vessel changes or prostate surgery

When Quad Mix May Not Be Right

Injection therapy isn’t for everyone, so we review your history before prescribing. Quad mix injections may not be a good fit, or may need extra care, if you have:

  • A blood disorder such as sickle cell disease, leukemia or multiple myeloma (higher risk of a prolonged erection)
  • An implant in the penis, Peyronie’s curvature or old scarring
  • A blood thinner, which makes bruising more likely
  • Heart disease that makes sexual activity unsafe right now
  • An allergy to any ingredient in the mix

Bring a list of every prescription, supplement and over-the-counter medicine you take. Nitrates and some blood pressure medicines matter here.

What to Expect at Bliss MD

Here’s how quad mix injections start at either clinic:

  1. Free discovery call. Fifteen minutes, by phone, to talk through your goals in confidence.
  2. A full first evaluation. We look at your hormones, blood sugar and diabetes risk, chronic conditions and weight. ED is often tied to them, and it can be an early sign of blood vessel problems.
  3. A supervised test dose. The first injection is the smallest one, and we titrate upward slowly while watching your response. An erection that overstays is treated on the spot.
  4. Hands-on training. You’ll practice drawing up and giving your dose, and learn what to steer clear of. You pick the delivery: a regular syringe or an auto-injector.
  5. Your monthly program. We supply the needles and syringes, alcohol pads and a sharps container.
  6. Fine-tuning. We adjust your dose based on your response, aiming for the lowest dose that does the job.

How Quad Mix Dosing Works

No two men need the same amount, so there’s no standard dose. Your physician chooses the strength of your mix. The amount you draw up is set during your test dose. Too little gives a weak response; too much raises the risk of an erection that won’t go down.

Our usual ceiling is two to three doses a week, with at least 24 hours between them. If one doesn’t work, skip the second try and phone us; we’ll rework it with you. The full walkthrough is in our dosing and side effects guide.

Side Effects and Safety

Most side effects of quad mix injections are mild. Careful dosing and good technique go a long way toward minimizing side effects. You may notice:

  • Brief stinging at the needle, or an ache linked to the alprostadil
  • A little bruising
  • Feeling flushed or light-headed
  • Over years of use, firm spots that can turn into scar tissue

Never pair quad mix with sildenafil (Viagra), tadalafil (Cialis) or PT-141 on the same occasion. Together they make a prolonged erection more likely.

Four hours is the hard limit. An erection that hasn’t gone down by then is an emergency, so go to the nearest ER. Before that point, ice can help, because cold narrows the blood vessels, and sometimes an antihistamine. We go over both at training; neither replaces the ER. The American Urological Association calls for prompt treatment of priapism.

How Much Do Quad Mix Injections Cost?

We don’t post prices, because each plan is individual. Your first visit is a full evaluation, and it’s separate. After that, quad mix is a monthly program that covers your medication plus visits and lab work as needed. Trimix is set up the same way.

Quad mix is a compounded prescription medicine for erectile dysfunction. It contains the three trimix medicines (papaverine, phentolamine and alprostadil) plus a fourth ingredient. A small amount is injected into the side of the penis, where it relaxes the erectile tissue and lets more blood flow in.

Most men notice an erection within about 5 to 15 minutes. Arousal, circulation and your dose all affect the timing. Your test dose in our office shows how you respond.
