Trimix or quad mix injections are usually the next step when ED pills haven’t worked well enough. Both are compounded medicines injected into the side of the penis, and both act within minutes. What separates them is one added ingredient, and how your body responds.
This guide covers how the two compare, where bimix fits and how a physician chooses. At Bliss MD, all three are offered at our Lincoln Park and Hanover Park clinics.

Trimix or Quad Mix Injections: The Short Answer
Trimix combines papaverine, phentolamine and alprostadil. Quad mix keeps those three and adds a fourth ingredient.
Most men who start injection therapy begin with trimix, as Memorial Sloan Kettering notes for its own patients. Quad mix tends to come up when a well-adjusted trimix dose still doesn’t give a reliable erection. Neither one wins for everyone. How you respond at your test dose guides the choice.
Trimix Injections for ED
Trimix is the most widely used injection for ED. Its three medicines relax the erectile tissue and open the arteries, so blood can flow in and stay. The American Urological Association lists injection therapy among the established ED treatments.
Trimix may suit you if:
- ED pills haven’t worked reliably, or cause side effects
- You take a medicine that shouldn’t be paired with ED pills
- ED is linked to diabetes, blood vessel changes or prostate surgery
Many men find a trimix dose that works well, and never need anything else. Trimix is usually kept in the refrigerator.
Quad Mix Injections for ED
Quad mix starts from the trimix formula and adds a fourth ingredient. It’s usually considered when trimix, at a carefully adjusted dose, still doesn’t produce a firm enough erection. Some physicians also try it when a man’s response to trimix fades over the years.
For some men, quad mix adds the support they need. For others, changing the trimix strength or amount is enough. If you’re weighing quad mix for ED, read about quad mix injections in Chicago and how treatment starts.
Where Bimix Fits
Bimix uses just two medicines, papaverine and phentolamine, and leaves out alprostadil. That can help men who feel an ache from alprostadil.
Bliss MD offers bimix too, so your physician can match the formula to how you respond.
Quadmix vs Trimix Side by Side
| Bimix | Trimix | Quad mix | |
|---|---|---|---|
| Medicines | Papaverine, phentolamine | Papaverine, phentolamine, alprostadil | The trimix three plus a fourth ingredient |
| Typical start | Minutes after injecting | About 5 to 15 minutes | About 5 to 15 minutes |
| Often considered | When alprostadil causes an ache | First, when pills fall short | When trimix needs more support |
| FDA status | Compounded, not FDA-approved | Compounded, not FDA-approved | Compounded, not FDA-approved |
These are typical patterns, not promises. Your dose, your health and your goals shape how any mix works for you.
How Fast Do Trimix and Quad Mix Work?
Trimix or quad mix injections work within minutes, not hours. Most men notice an erection about 5 to 15 minutes after the injection. Arousal helps, and the medicine tends to work better if you stand, says UCSF Health.
With a well-set dose, the erection usually lasts about 30 minutes to an hour, then fades on its own. If it lasts much longer, your dose needs a change. An erection lasting more than 4 hours is a medical emergency.
Switching From Trimix to Quad Mix
With trimix or quad mix injections, the choice is often a step, not a starting point. If trimix isn’t giving you the response you want, don’t raise the dose on your own. Call your physician first. Often the first fix is a change in trimix strength or amount.
If quad mix is the next step, it starts in the office. A new formula means a new supervised test dose at the smallest amount, then slow titration. Read more about quad mix dosage, test dose and side effects.
Side Effects of Trimix and Quad Mix
Trimix or quad mix injections share the same main side effects. Careful dosing and good technique help with minimizing side effects:
- A brief pinch, or a dull ache afterward (often from alprostadil)
- Small bruises at the injection site
- Light-headedness or flushing
- With long-term use, firm areas or scar tissue in the penis
Don’t use either mix at the same time as sildenafil, tadalafil or PT-141. Never inject more than once in 24 hours, and don’t take a second dose if the first one doesn’t work. Memorial Sloan Kettering gives its patients the same rules.
If an erection lasts more than 4 hours, go to the nearest emergency room.
Do Trimix and Quad Mix Cost the Same?
At Bliss MD, trimix or quad mix injections are handled the same way. After a separate first evaluation, either mix is part of a monthly program covering your medication plus visits and lab work as needed. We don’t post prices; your plan sets your number.
Insurance usually doesn’t cover these medicines, and we don’t bill insurance. HSA, FSA and CareCredit financing are options. Read more about quad mix cost.
How We Choose Between Them at Bliss MD
Choosing between trimix or quad mix injections is done together, after a full first evaluation with Dr. Anand Thakkar’s team. We look at your hormones, blood sugar, chronic conditions, weight and medicines. Then we recommend bimix, trimix or quad mix and set its strength.
Your first injection is a supervised test dose in our office. You’ll learn how to dose, what to avoid, and whether you’d prefer a standard syringe or an auto-injector. Mixes come from U.S.-based, FDA-inspected compounding pharmacies. Learn more about erectile dysfunction treatment or how low testosterone and ED connect.
Trimix vs Quad Mix FAQs
Trimix has three medicines: papaverine, phentolamine and alprostadil. Quad mix contains the same three plus a fourth ingredient. Both are compounded ED injections, and both are dosed for you.
Not for every man. Quad mix is usually considered when a well-adjusted trimix dose still isn’t enough. Many men do well on trimix and never need to switch.
Within minutes, not hours. Most men notice an erection about 5 to 15 minutes after the injection, with either mix.
With a well-set dose, an erection often lasts about 30 minutes to an hour. If one lasts more than 4 hours, go to the nearest emergency room.
The common ones are shared: a brief pinch, a dull ache and small bruises. The most important risk is a prolonged erection, which is why your dose starts small and rises slowly.
Bliss MD generally recommends no more than 2–3 times a week, and never more than once in 24 hours, for either mix. Don’t re-inject if a dose doesn’t work.
Yes, if your physician recommends it. A new formula means a new supervised test dose, so the switch starts in our office.
